Make a referral

For GPs, support coordinators, allied health professionals and anyone referring on behalf of someone else.

What happens next

We'll contact the person, or their representative, to talk about their needs, how sessions will be funded and when we can start. We'll let you know once we've made contact.

If we can't help, for example because of where they live or how their plan is managed, we'll tell you and suggest other options.

What to include

Only what we need to get in touch and understand the request: contact details, a sentence or two about why you're referring, and how therapy will be funded.

Please leave out detailed medical historyIf you have reports, plans or letters to share, we'll send you a secure way to provide them once we've made contact. Please don't attach them to an ordinary email.

Before you refer

Please make sure the person knows about the referral and is happy for us to contact them. For NDIS participants, let us know how their plan is managed.
